Limitations

  • Only works on PDFs that already have AcroForm fields built in. A scanned paper form with no interactive fields needs Fill and Sign PDF instead, which lets you place text anywhere.

Those lines are probably just drawn graphics, not real form fields (no AcroForm data). Use Fill and Sign PDF instead, which lets you click anywhere on the page to place text or a signature.
